By checking the box below, I hereby agree to indemnify and hold harmless Colborne Street United Church of London, Ontario, Canada, and its agents, employees, volunteers, and representatives, from all liability relating to injuries that may occur while attending or participating in, whether as a player or spectator, any sports activities played on the premises of Colborne Street United Church. By signing this agreement, I agree to hold Colborne Street United Church entirely free from any liability, including financial responsibility, for injuries incurred, regardless of severity and regardless of whether injuries are caused by negligence, oversight, or error.
I also acknowledge the risks involved in playing sports. These include but are not limited to tripping, slipping, falling, colliding with another player, being struck by a ball, racquet, stick or other sporting equipment, whether worn or held, over-extension of muscles, and the sudden onset of any unknown underlying health conditions. I swear that I am participating voluntarily, and that risk of injury and relevant safety precautions have been made clear to me verbally or in writing. Additionally, I acknowledge that I do not have any health-related conditions that will increase my likelihood of experiencing injuries while engaging in sports activities at Colborne Street United Church.
I understand I am forfeiting all right to bring a suit against Colborne Street United Church for any reason relating to attending a sports event on the Colborne Street United Church premises. I also agree to make every effort to obey safety precautions as listed in writing and/or as explained to me verbally and to ask for clarification when needed.
